Revitalizing School Libraries:
Using Program Evaluation Techniques to Change Your Practice

The following are links to tools used by the Archdiocese of New York Library Connections program staff, participating schools, and program evaluators.

Goals and Objectives (in Word format)
The Library Connections program is organized around a set of key goals and objectives. Many of the evaluation tools developed for the program also use these as their framework.

Management Tool
The management tool is filled out twice yearly by library teachers and principals. The document completed in the fall asks participants to set goals for the year. In the spring the tool asks for data that gives schools the opportunity to reflect on how well they met their goals.

Mentor Reporting Form (in Word format)
Each school in the program is assigned a mentor. These mentors fill out and submit reports throughout the year that help gauge the progress and success of each school in the program.

Scope and Sequence (in Word format)
Library Connections staff along with staff in participating schools developed a library skills scope and sequence.

Unit Plan Form (in Word format)
This is the form that library teachers and faculty members fill out and submit to Library Connections. It is used to demonstrate and record collaborative efforts by school staff.
